 Obtaining a Collision Report
When the Accident Happened in
Canyon Lake, Perris, and Winchester CA 

CHP Assisted?

If the Collision occurred on the Freeway or on a Highway where the California Highway Patrol assisted in the report, then you will need to contact the CHP office:

For Wildomar or Lake Elsinore you will need to contact the Temecula CHP Office and verify if the report is ready:

​

If it is ready:

 

You can go into that office and personally pick it up

 

OR

 

You can have it mailed to your address.

 

  1. Ask the person on the phone how much the report will cost (costs vary depending on how many pages, usually $10.00)

  2. Write a check made payable to CHP for that amount.

  3. Fill out the form below

  4. Photocopy your driver license or picture identification card

  5. Send the above to this address:

Sheriff or Police  Assisted?

If the Collision occurred on a street where the Sheriff Department assisted in the report, then you will need to contact the Riverside County Sheriff Department:

For Canyon Lake, Perris, and Winchester, you will need to contact the Perris Office and verify if the report is ready:

​

If it is ready:

 

You can go into that office and personally pick it up

 

OR

 

You can have it mailed to your address.

 

  1. Ask the person on the phone how much the report will cost and to whom the check should be payable to. 

  2. Write a check made payable to that department or city
